Imaging of Soft Tissue and Bone Tumor and Mimics in Pediatric Hands and Feet

Purpose or Case Report: Bone and soft tissue tumor in pediatric hands and feet are not infrequently encountered in pediatric patient. The lesion can be benign and malignant. Some are specific on imaging and some are nonspecific requiring biopsy. Recognizing imaging specific findings and correctly triage the patient is important in patient management.
Methods & Materials: This will be a case-based presentation including the following categories:
1.Aggressive bone lesions
2.Benign bone lesions
3.Malignant soft tissue tumors
4.Benign soft tissue lesions
5.Calcified lesions
6.Vascular lesions
7.Fatty tumors
Results: The cases included in this exhibit include:
1. Aggressive bone lesions: osteomyelitis, undifferentiated sarcoma
2. Benign bone lesions: aneurysmal bone cyst, simple bone cyst, giant cell reparative granuloma, giant cell tumor, hemangioma, enchondroma, Trevor's disease, osteoid osteoma, osteoblastoma, abscess
3. Malignant soft tissue tumors: infantile fibrosarcoma, rhabdomyosarcoma, synovial sarcoma, other soft tissue sarcomas including MPNT
4. Benign soft tissue lesions: ganglion cyst, desmoid tumor, giant cell tumor of tendon sheath, infantile digital fibromatosis, myofibroma, nerve sheath tumor, dermatofibroma, granuloma annulare
5. Calcified lesions: synovial sarcoma, soft tissue chondroma, venous malformation, chondroosseous lipoma
6. Vascular lesions: AVM, venous malformation, lymphatic malformation, hemangioma, Kaposiform hemangioendothelioma
7. Fatty tumors: lipoma, fibrolipomatous hamartoma of nerve
Conclusions: 1. Malignant bone lesions are rare in the hands and feet, but certain imaging features can suggest aggressiveness and, therefore, a need for biopsy.
2. Soft tissue masses often show nonspecific imaging findings and require a tissue diagnosis.
3. Knowledge of the spectrum and imaging features of benign and malignant bone and soft tissue lesions in the hands and feet in pediatric patients is crucial for assessment and management of these pediatric lumps and bumps.
