Role of Doppler Ultrasound in diagnosis and treatment of a Femoral artery Pseudoaneurysm in a Pediatric patient

Purpose or Case Report: To demonstrate the finding of a Femoral artery Pseudoaneurysm on a Left Lower extremity venous and arterial doppler ultrasound and under Interventional Radiology sonographically guided compression of the neck of the Pseudoaneurysm and guided injection of thrombin into the Pseudoaneurysm resulted in successful thrombosis of a Pseudoaneurysm. Follow up ultrasounds to document resolving of a Left Femoral artery Pseudoaneurysm in an infant.
Methods & Materials: 3 months old patient with complex history of Cyanotic heart disease presented to our Emergency room with hypoxia, episodes of cyanosis at home and loss of appetite. Bilateral Lower Extremity venous doppler was ordered to rule out deep vein thrombosis. Overall exam was negative
but there were two vascular structures demonstrated in the Left groin with arterial waveform and continous with left Iliac artery. After two weeks, another Left unilateral Lower extremity arterial doppler was ordered for pulsatile mass in the left groin. Rapidly, enlarging 2cm Pseudoaneurysm of the Left femoral artery was demonstrated on a doppler ultrasound. Patient was transferd to an Interventional Radiology department for Ultrasound guided compression of the neck of the Pseudoaneurysm for twenty minutes and Thrombin injection was inserted into the lesion.
Results: Documentation of series of Left lower extremity arterial doppler ultrasounds demonstrating a successful resolution of a Left femoral artery Pseudoaneurysm after the Interventional procedure.
Conclusions: Ultrasound is one of the best preferred imaging modality used toward the pediatric population as a screening tool, a guidance during a complicated Interventional procedures and also used as a follow-up imaging modality for resolution of a femoral artery Pseudoaneurysm.
