The long term effects of retained Gadolinium (Gd) are still under investigation, but may be of greater concern in pediatric patients. So far several factors that appear to influence Gd retention have been described. The chemical structure of the Gadolinium based contrast agents (GBCA) plays an important role, with linear agents showing greater deposition compared with macrocyclics, attributed to lower chelate affinity. Differences in GBCA washout may also play a role. Fallopian tube torsion is a rare cause of acute abdominal pain in pediatric patients. Pelvic ultrasound is the first imaging modality utilized to evaluate for pelvic pathology, typically to rule out ovarian torsion. Ultrasound findings of fallopian tube torsion can be challenging, leading to delayed surgical intervention, particularly when ovaries have a normal gray scale appearance and spectral Doppler waveforms. Liver transplantation is a potentially life-saving procedure for children with liver failure. Pediatric liver transplant recipients are at greater risk for complications such as vascular thrombosis than adults. In addition, children more commonly receive reduced size grafts, which alter the normal liver anatomy. While the postoperative assessment of pediatric liver transplants with sonography has been well studied, less exists in the radiology literature regarding intraoperative Doppler sonography of the graft prior to abdominal closure. We illustrate the intra- and post-operative ultrasound findings in pediatric liver transplants, with an emphasis on normal features, common pitfalls, and frequently encountered vascular complications.
